Terms and Conditions
Review draft — not an adopted production policy.
Bracketed instructions identify business facts or implementation choices that must be completed and removed before adoption. This template requires legal review for the actual operations and launch states. The current prototype does not submit answers, send verification texts, or collect actual marketing consent. Hosting infrastructure may process request logs. Proposed production practices below are not a claim that those integrations are active.

1. About these terms
These Terms and Conditions govern your use of the websites and inquiry forms operated by Altitude Leads LLC (“Altitude Leads,” “we,” “us,” or “our”) that link to these Terms. Our consumer-facing page names may differ; the operator remains Altitude Leads LLC.
Please review these Terms before using our services. Where acceptance is required, we will present an appropriate acceptance mechanism. If you do not agree, do not submit an inquiry. These Terms do not replace the terms of any insurance policy or create marketing consent by themselves.
2. Our role
Altitude Leads provides insurance-related information and connects interested individuals with insurance providers and licensed professionals. We are not an insurer and do not issue policies, underwrite coverage, adjudicate claims, or bind an insurer. [CONFIRM: the company’s role and licensing status before adoption.]
We may be paid for referrals or lead delivery. We do not necessarily present every insurer or product, and our service does not promise a complete market comparison. A “benefits advocate” describes the person assisting with an inquiry; it does not imply government affiliation or an independent fiduciary relationship.
3. Eligibility and accurate information
The service is intended for U.S. residents age 18 or older. Submit only your own information, or information you are legally authorized to provide. Information must be accurate to the best of your knowledge. You must be authorized to use any phone number or email address you provide.
Do not impersonate another person, submit unauthorized or automated inquiries, misuse verification codes, interfere with the service, attempt unauthorized access, or use the site for unlawful activity. We may restrict misuse, subject to applicable law.
4. Inquiries, quotes, and insurance coverage
Submitting a form starts an inquiry. It is not an insurance application, approval, purchase, or enrollment, and it does not create coverage. Phone verification confirms access to a phone number; it does not establish eligibility or approval.
A “breakdown,” “quote,” or “options” refers to information supplied through the provider’s process. Unless expressly identified otherwise, estimates are preliminary and subject to verification. We do not guarantee an instant price, the lowest rate, approval, or availability.
Availability, premiums, benefits, exclusions, waiting periods, and underwriting requirements vary by provider, product, state, and applicant. Only the insurer’s issued policy and applicable documents determine coverage. Do not cancel existing coverage based solely on a website statement or inquiry.
5. Independent providers and third-party services
Insurance products and services are provided by independent providers under their own terms and privacy notices. Review their credentials and documents before making a decision. Altitude Leads is not responsible for a provider’s independent coverage decisions or performance, except where applicable law provides otherwise.
Links to third-party websites are provided for convenience and do not imply endorsement. This website is not part of, sponsored by, or endorsed by Facebook or Meta. Third-party names and trademarks belong to their respective owners.
6. Communications and consent
Submitting an inquiry may involve a separate disclosure about calls, texts, and emails from identified parties. Marketing consent is governed by the disclosure and affirmative action presented at that time, not solely by these Terms.
Where expressly authorized, communications may use automated dialing or artificial, prerecorded, or AI-generated voices. Consent is not a condition of purchasing insurance. You may decline or revoke marketing consent as described in the form disclosure and Privacy Policy.
If a phone-verification service is enabled, requesting a code authorizes the verification message for that request; it does not by itself authorize marketing. Message and data rates may apply. Messages may be delayed or undelivered due to network or carrier conditions.
For a supported SMS program, reply STOP to opt out or HELP for assistance; message frequency varies. You may also contact [SUPPORT EMAIL / PHONE] or use an email’s unsubscribe option. [CONFIRM: program name, actual sender, supported commands, suppression procedures, and partner opt-out handling before launch.]
7. Privacy
Our Privacy Policy explains information collection, use, disclosure, and available choices. Health-related answers may be sensitive. Do not submit unnecessary medical records, payment information, or Social Security numbers through the initial inquiry form.
Acceptance of these Terms is not a substitute for any separate consent or notice required for sensitive information, marketing, or tracking.
8. Website content and permitted use
Unless otherwise stated, site content, branding, and design are owned by Altitude Leads or its licensors. You may view and use the site for personal, lawful insurance research. Do not reproduce or commercially exploit protected content without permission, except as permitted by law.
General website information is educational and is not a substitute for individualized advice from an appropriately licensed professional. We may correct errors or update content. You remain responsible for reviewing actual provider and policy documents.
9. Availability and limitations
To the extent permitted by law, the website is provided “as is” and “as available,” without warranties that it will be uninterrupted, error-free, or suitable for every purpose. We do not guarantee that any provider will contact you or offer coverage.
To the extent permitted by law, Altitude Leads is not liable for indirect, incidental, special, or consequential losses arising from use of the website. Nothing in these Terms excludes liability that cannot lawfully be excluded, or limits nonwaivable consumer rights. [LEGAL REVIEW: confirm enforceability and any additional limitations for the intended jurisdictions. No fixed damages cap is included in this template.]
10. Disputes and applicable law
For questions or concerns, contact [LEGAL / SUPPORT EMAIL]. This contact process does not prevent you from exercising statutory rights or seeking available remedies.
[INSERT: approved governing-law and venue provisions consistent with the operator’s jurisdiction and applicable consumer protections.] This draft does not include mandatory arbitration or a class-action waiver. Any such provision would require a separately considered and properly implemented agreement.
11. Changes, general provisions, and contact
We may revise these Terms and post an updated effective date. Material changes will be communicated, and renewed agreement obtained, where required. Changes do not retroactively remove accrued rights.
If a provision is unenforceable, the remaining provisions continue to the extent permitted by law. A failure to enforce a provision does not waive it. Nothing here creates an employment, partnership, or agency relationship with a visitor.
