Privacy Policy
Review draft — not an adopted production policy.
Bracketed instructions identify business facts or implementation choices that must be completed and removed before adoption. This template requires legal review for the actual operations and launch states. The current prototype does not submit answers, send verification texts, or collect actual marketing consent. Hosting infrastructure may process request logs. Proposed production practices below are not a claim that those integrations are active.

1. Who we are and where this policy applies
Altitude Leads LLC (“Altitude Leads,” “we,” “us,” or “our”) operates consumer-facing websites and landing pages that help adults request information about life insurance, including final expense insurance. This policy applies to the websites and forms that identify Altitude Leads LLC as their operator and link to this policy. Different page names do not change the operator.
Covered brands currently include Seniors With a Plan, Simple Wishes Circle, Plainspoken Seniors, Faithful Senior Circle, and Life Beyond the Road. Independent insurance providers have their own privacy notices, which govern their handling of information after they receive it.
2. Information we collect
Information you provide may include your name, email address, phone number, ZIP code, date of birth, desired coverage amount, marital status, and your answer to the question about a heart attack, stroke, or cancer within the last three years. That answer is health-related information and may be sensitive personal information.
When enabled, phone verification and consent records may include verification status, timestamps, the disclosure presented, the action you took, and technical information used to document the request. Do not submit Social Security numbers, payment card details, medical records, or information about another person through our initial inquiry form.
Website infrastructure may process IP addresses, browser and device information, pages requested, referral sources, and security logs. [CONFIRM: actual hosting logs, cookies, pixels, analytics, advertising identifiers, and consent-recording vendors before launch.]
3. Where information comes from and how we use it
We receive information directly from you and from the website technologies described in this policy. Where enabled, service providers may return phone-verification, fraud-prevention, and lead-delivery status.
We use information to process your request, identify relevant insurance-provider options, verify contact information, route your inquiry, respond to questions, document your choices, prevent misuse, and comply with applicable obligations. We may use limited operational data to measure and improve the service.
Marketing communications depend on the consent and choices presented to you. Reading this policy does not itself authorize marketing calls, texts, or emails. [CONFIRM: any partner feedback, call recordings, enriched data, or additional uses; add them here if used.]
4. How information is shared
We share the information needed to respond to your request with the insurance agent, agency, provider, or distribution partner identified in the applicable form disclosure. Recipients may include licensed insurance professionals who contact you about your inquiry. [INSERT: recipient identities or a current partner disclosure and describe any downstream lead distribution.]
We may receive compensation for referrals or lead delivery. Such transfers can constitute a “sale” under some state privacy laws. [CONFIRM: actual compensation and data transfers, applicable legal definitions, and required opt-out mechanisms. Do not replace this section with a blanket “we never sell data” statement without verification.]
We also share necessary information with contracted service providers for hosting, security, communications, verification, and lead management; with authorities or advisers when legally required or necessary to protect rights; and in connection with a business transaction, subject to applicable law.
Health responses require separate consideration before sharing or advertising use. [CONFIRM: recipients, purposes, restrictions, and any required separate health-data notice or consent. Do not enable health-data advertising transfers based on this template.]
5. Cookies, analytics, and advertising
[COMPLETE BEFORE LAUNCH: identify the technologies actually enabled, their purposes, the data sent, the recipients, and how visitors can manage them.]
If advertising or analytics tools are enabled, describe them here and distinguish essential service operations from optional tracking. State whether transfers constitute sale, sharing, or targeted advertising under applicable law. Provide a working privacy-choice mechanism and implement consent or opt-out preference signals where required. Browser cookie settings alone may not cover server-side transfers.
The current review prototype has no configured application analytics, advertising pixels, form transmission, or browser storage for answers. Hosting infrastructure may still process request logs. This statement must be updated when production integrations change.
6. Calls, texts, emails, and verification
If you choose to provide the consent shown on a live form, the identified parties may contact you within the scope of that consent. Any authorization for automated dialing, artificial or prerecorded voices, or AI-generated voices must be stated in the applicable disclosure; this policy does not create that authorization.
A verification text confirms access to a phone number and is separate from marketing permission. Message and data rates may apply. Where SMS is offered, message frequency varies.
You can request that marketing calls stop, use the unsubscribe option in marketing emails, or reply STOP to a supported messaging program. For assistance, use [SUPPORT EMAIL / PHONE]; an enabled SMS program should also support HELP. We will process valid requests as required by applicable law. A necessary confirmation or nonmarketing service message may still be permitted. Providers independently holding your information may have their own request channels. [CONFIRM: working suppression, STOP/HELP, partner notification, and support processes.]
7. Retention and security
We retain personal information only as reasonably needed for the purposes described, including responding to requests, maintaining consent and suppression records, resolving disputes, preventing fraud, and meeting legal obligations. [INSERT: approved retention periods or specific criteria for inquiry data, health answers, verification records, consent evidence, and logs.]
We use reasonable administrative, technical, and organizational measures appropriate to the information we handle. No internet service can guarantee absolute security. [CONFIRM: these safeguards match implemented access controls, encryption, vendor arrangements, and deletion processes.]
8. Your privacy choices and rights
Depending on your location and applicable law, you may have rights to access, correct, delete, or obtain a copy of personal information; opt out of sale, sharing, or targeted advertising; limit certain sensitive-data uses; withdraw consent; or appeal a denied request.
Submit a request through [PRIVACY EMAIL] or [PRIVACY REQUEST URL / PHONE]. We may need proportionate information to verify identity or an authorized agent’s authority. We will respond within applicable deadlines and explain any permitted exception or denial. To appeal where available, use [APPEAL CONTACT / METHOD]. We will not unlawfully discriminate against you for exercising privacy rights.
[COMPLETE: applicable state supplements, collection/recipient tables and lookback disclosures, sale/sharing categories, sensitive-data treatment, Global Privacy Control handling, request channels, and appeal instructions. Review consumer-health-data rules for every launch state. This template alone does not implement these controls.]
9. Children, other websites, and changes
Our service is intended for adults age 18 or older. We do not knowingly solicit personal information from children. Contact us if you believe a child has provided information so we can take appropriate action.
Other websites and independent providers are responsible for their own practices. Our services are intended for U.S. residents. [CONFIRM: vendor processing locations and disclose cross-border processing if relevant.]
We may update this policy and will identify the effective date. For material changes, we will provide additional notice or obtain consent when required. Posting an update does not automatically authorize a new use of previously collected information.
